Your medical record contains a lot of information about you, including lab results, medication lists, visit summaries, and X-ray reports. The medical record also includes information. They record the conversation you had with your doctor, nurse or other health professional and contain a summary of the most important information discussed. This patient visited an orthopedic specialist to discuss a hip replacement. In the note, the doctor describes the visit and the patient’s symptoms. The doctor also explains the patient’s next steps, including follow-up examinations. The patient uses the letter to remind himself of the appointment he has to make. She also shared the letter with a supportive family member and kept the letter on file at home in case she wanted a second opinion.
This patient has weekly therapy meetings with a social worker to discuss her feelings of depression and anxiety, which have worsened since a recent diagnosis of Parkinson’s disease. In the note, the therapist explains the patient’s physical and mental health symptoms and describes some of the coping strategies that were discussed in the therapy session. This patient was relieved to read that her specialist understood her condition. Parkinson’s affects his memory, so being able to review the notes helps him remember what he needs to do between visits. Additionally, the patient can print out a letter and take it to the prescriber so that all members of his or her health care team are informed about his or her care.
This patient has type 1 diabetes and at least two other chronic diseases. He takes several medications and uses his writing regularly to manage his health. He recalled a time when the medication dosage seemed incorrect when he went to pick it up at the pharmacy. He checked his notes to remember what the doctor had told him and was able to adjust the dose. Additionally, the endocrinologist involved in her diabetes treatment is not at the same hospital where she receives her other care, but can post information and share it. It is important to him that his entire healthcare team has access to the same information. Writing has helped her feel like an expert on her own condition, and she says that despite her difficult medical life, she feels healthy.
What Does A Visit Note Look Like?
To prepare for surgery, this patient visited an otolaryngologist (ear-nose-throat) to determine if a breathing tube could be placed in his airway during surgery. The note summarizes the patient’s medical history and describes the diagnosis. The message contains complex medical terms, e.g.
. The patient said he didn’t understand the words but still liked the letter. He looked up certain words on the Internet and wrote a list of questions in preparation for surgery. He was also able to share the letter with his primary care physician at another institution.

The doctor’s note should include the date you saw the doctor that you had a good reason to be off work, any restrictions they recommended, and whether any time off work is required. Remember that due to doctor-patient confidentiality, they cannot disclose your illness without your permission.
