Signup Sheet Template – An access sheet or registration sheet is used to record information about attendees or guests of an event. When entering the venue, participants will have a form to write their name, time (with date) and signature (if required). The form can also be used to collect information about visitors, both for contact and for the person to be present at a particular event.
Due to the limited area of an 8.5″ by 11″ sheet of paper, it is best to limit the sheet to a maximum of five (5) columns. Especially if you get the person’s email, phone or mailing address.
